Dose determining weight (DDW)
Aminoglycosides (amikacin, gentamicin, tobramycin) distribute poorly into adipose tissue. Some adjustment for this needs to be made if the patient is obese. A dose determining weight should be used and is calculated from ideal body weight (IBW).
Formula
IBW for males = 50 + ( 2.3 x ( height in inches - 60 ) )
IBW for females = 45 + ( 2.3 x ( height in inches - 60 ) )
DDW = IBW + 0.4 x (actual body weight (kg) - IBW )
